The scope of controlled drugs that requires a special prescription for controlled drugs in schedules I through III approved and promulgated by the Executive Yuan. |

Special prescriptions for controlled drugs are to be handwritten by the physician or dentist and handed to the patient for dispensing at the pharmacy. Content of the prescription should contain the prescription issued date; patient’s name, sex, date of birth, dentification number, and medical record number; name of disease; name, specifications, quantity, usage, and unit of controlled drug; number of days of single dispensation, total prescription amount of single dispensation, and range of total prescription amount of single dispensation; name of prescribing medical institution; prescribing physician’s/dentist’s controlled drug use license number, telephone number and signature, and the prescription allows for times of dispensing; dispenser’s professional certificate number and signature, dispensing date, name of dispensary; signature of recipient and patient’s telephone number. Format of the prescription is as shown in ppendix |

The content of special prescription for controlled drugs is keyed in the computer and printed out by the physician/dentist, then handed to the dispenser.
The prescribing medical institution may establish its own format for the prescription. The content of prescription is not necessary to contain the range of total prescription
amount of single dispensation. |

If controlled drugs in schedules I through III are prescribed and dispensed in the same medical institution and are provided for the use of patients in the hospital during hospitalization or surgery. The medical institution may establish its own format of the special prescription. The content of prescription is not necessary to contain the range of total prescription amount of single dispensation,prescribing medical institution’s name, prescribing physician’s/dentist’s telephone number, the prescription allows for times of dispensing, name of dispensary and patient’s telephone number. |
